Traveler is in FITC prior to submitting this form (Concur imports into FITC for payment)
Supplier profile in FITC must have Supplier Type of Student Traveler or Employee (ensures the supplier has a TER site which drives payment)

When NOT to use this form

If traveler is in Concur-but needs Supplier ID added. Do not submit this form, instead send email request to fsku@ku.edu
To add/update a Supplier to FITC.
To add an employee to Concur. KU Employees are added through an HR import. If there is an employee that needs to be added to Concur, do not submit this form, instead send an email request to fsku@ku.edu
Reimbursement for Recruitment (submit FITC as a Non PO Invoice)
To add a KU –Affiliate that will only be receiving one reimbursement (submit Non-PO Invoice in FITC)
